We will use the information you give us to choose the most suitable doctor, nurse, health professional or service to help you
Any appointment requests, for children under 12, will be reviewed by our team on the day. We will then offer you an appointment if necessary. We may re-direct you to a different service on the same day
If possible please try to telephone Reception before 08:00 if you require a home visit.
You may only request a home visit if you are housebound. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.
You can be visited at home by a Community Nurse if you are referred by your GP. You should also be visited at home by a Health Visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else. Please remember not cancelling your appointment can result in other people missing out. Please make sure you cancel as soon as possible so it can be given to someone else.
You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance (excluding weekends and public holidays). You can also text to cancel your appointment.

Good news! The six practices in the Pennines PCN—Littleborough Group Practice, Milnrow Village Practice, Pennine Surgery, Stonefield Street Surgery, Trinity Medical Centre, and The Village Medical Centre have teamed up to make accessing care outside normal hours easier than ever.
We have designed an Extended Access Service with you in mind, offering appointments across both Littleborough and Milnrow. That means more choice, better access, and less travelling when you are not feeling your best.

You will have access to a friendly team of healthcare professionals, including GPs, Practice Nurses, Nursing Associates, Healthcare Assistants, and Phlebotomists, ready to help when you need them.
